Care
Management:
This is a program provided directly by
Region IIIC Area Agency on Aging. It
fulfills clients' wishes to remain in their
own home by arranging in-home long-term care
services when they are at risk or in need of
nursing home placement. The program is
available to persons who are:

60 years of age and older
-
Medically complex
-
Currently a resident of a skilled or
intermediate facility no longer
requiring that level of care, but is
unable to obtain the needed community
services to remain home
-
Currently at home, but considering a
move to another location
-
At risk or in need of nursing home
placement
Some of the services arranged by Care
Management include:

Home Delivered Meals
-
Personal Care
-
Respite Care
-
Light Housekeeping
-
Personal Emergency Response Systems
-
Medication Management and Assistance
-
Referral to other appropriate agencies
if Care Management is no longer able to
meet the needs of the client
